　われわれは強直性脊椎炎に第7頚椎骨折を合併した1例を経験したので報告する．症例は56歳の男性で，転倒して受傷した．多くの場合，強直性脊椎炎に伴う脊椎骨折に対しては重度の不安定性のため手術が行われているが，従来の固定法では十分な固定を得ることが困難である．本症例においては強力な固定力を得るためMagerl法，椎弓ワイヤリング，椎弓根スクリューを用いてC2-Th10の後方固定を行った．術後，頚部痛は軽減し，良好な骨癒合を得ることができた．

　We report the case of a 56-year-old man with ankylosing spondylitis who experienced a C7 fracture. In most cases surgery is indicated because of severe instability, but it is difficult to achieve adequate fixation. Posterior fixation from C2 to Th10 by the Magerl's technique and with pedicle screws and sublaminar wiring was performed in our patient. The neck pain resolved, and bony fusion was achieved by 6 months postoperatively.
