　腰部脊柱管狭窄で手術を行った55例に対し，安静時症状と歩行で誘発される症状の改善度の差異について検討した．術後1年での安静時症状の残存率は57％であり，歩行時出現症状の残存率は31％であった．症状別では，下肢のしびれが残存しやすい傾向にあり，特に，安静時の下肢のしびれは腰痛と下肢痛に比して残存しやすかった．症状の残存に関与する因子は手術時年齢と馬尾障害の有無であった．腰部脊柱管狭窄の手術時には，術前に患者に対してこれらの点について十分に説明する必要がある．

　We compared the symptoms of 55 patients before and after spinal decompression with and without fusion for degenerative lumber spinal stenosis. At the 1-year follow-up, the proportion of postoperative residual symptoms at rest was 42～74％ (95％CI), and the proportion of residual symptoms when walking was 18～44％ (95％CI). The proportion of residual numbness in the lower limbs at rest is significantly higher compared with the proportion of low back pain and leg pain at rest. It is essential to inform patients these regards before operation to obtain their consent.
