背景:腰部脊柱管狭窄において,術前の整形外科患者に対する精神医学的問題評価のための簡易質問票(以下,BS-POP)と手術による症状の改善度との関係を検討した.
方法:腰仙椎部神経根障害に対して椎弓骨切り術を行った腰部脊柱管狭窄38例を対象とした.対象を術前のBS-POPで,正常群と異常群に分けて前向きに比較検討した.
結果:術前のBS-POPによる評価で,異常群は9例,正常群は29例であった.術後成績をみると,BS-POP異常群では,正常群と比較して,術後下肢痛numerical rating scale(NRS)が高く,下肢痛NRSの改善率が低く,下肢痛の残存が有意に多かった(p<0.05).すなわち,BS-POP異常群は,BS-POP正常群と比較して,手術による下肢痛の改善が得られにくいと言える.
結論:術前におけるBS-POPによる評価は手術成績不良例をスクリーニングできる可能性がある.
Background:The relationship between surgical outcomes and scores on the Brief Scale for Psychiatric Problems in Orthopaedic Patients (BS-POP) was evaluated in lumbar spinal stenosis patients.
Methods:Laminotomy was performed to treat 38 patients with lumbar radiculopathy. The subjects were divided into two groups according to their preoperative BS-POP scores and compared prospectively.
Results:Nine patients had preoperative BS-POP scores in the abnormal range, and 29 patients had BS-POP scores in the normal range. The group with BS-POP scores in the abnormal range had higher numerical rating scale (NRS) scores for leg pain, a lower leg pain recovery rate, and a higher residual leg pain rate than the group with BS-POP scores in the normal range.
Conclusion:The results of this study indicated that preoperative BS-POP scores predicted a poor outcome of surgical treatment for lumbar radiculopathy.
