高齢者における腰部脊柱管狭窄の術後QOL

Postoperative QOL of Elderly Lumbar Spinal Canal Stenosis Patients

　本研究の目的は，患者立脚アウトカムの指標であるRoland-Morris機能障害質問表（以下RDQ）を用いて，患者の立場からみた70歳以上の腰部脊柱管狭窄の術後QOLを明らかにすることである．腰部脊柱管狭窄と診断し，当科で手術を行い，術後3カ月以上経過した症例50例を検討の対象とした．以下の項目を検討し，70歳未満群と70歳以上群とで対比した．1）症状の有無とその程度，2）歩行可能時間，3）JOAスコア，4）手術に対する満足度，5）腰痛関連QOL，6）精神医学的問題の有無である．70歳以上群では，腰痛と下肢のしびれの程度が，70歳未満群に比べ有意に高く，JOAスコアは，70歳以上群のほうが70歳未満群よりも有意に低かった（p＜0.05）．腰痛関連QOLは，RDQの偏差得点で比較すると両群間で差はなかった．すなわち，両群とも術後の腰痛関連QOLは，全国の腰痛有訴者のRDQ基準値まで回復する．精神医学的問題を有する症例では，両群とも手術に対する満足度と腰痛関連QOLが明らかに低かった．精神医学的問題を有する症例は，年齢に関わらず手術に対する満足度と腰痛関連QOLが明らかに低いので，手術は可能な限り回避する必要がある．

　The aim of this study was to clarify the postoperative QOL of elderly lumbar canal stenosis patients by means of the Roland-Morris disability questionnaire (RDQ). The subjects were 50 patients with lumbar canal stenosis whose course was followed up for more than three months after surgery in our department. Their symptom intensity, gait capacity, JOA score, satisfaction, QOL related to low back pain, and psychiatric problems were assessed. There was no significant difference between the elderly group (70 years old and over) and the adult group (69 years and under) in the deviation value of the RDQ calculated based on reference values in Japan. Satisfaction and QOL related to low back pain were significantly low in patients with psychiatric problems in both groups.
