　本邦では，従来固定を要する腰椎疾患に対し腰椎前方固定術が一般的であった．しかし，椎弓根スクリューやインストゥルメンテーションの改良に伴い，神経組織の除圧と罹患椎間の固定を要する疾患に対し，腰椎後側方固定術や腰椎後方椎体間固定術を選択することが多くなってきている．一方，腰痛を主症状とする疾患に対しては，現在欧米を中心に椎間孔経由腰椎後方椎間固定術（TLIF）が施行されているが，左右いずれか一側から腰椎椎体間固定を達成できる術式であり，今後腰椎固定術のオプションになりうる可能性がある．そこで，本論文では自験例のTLIFの適応，手術手技，手術成績および腰椎固定術に対する文献的考察を行った．

　Anterior lumbar intervertebral fusion was formerly the gold standard for lumbar fusion in Japan, as a result of the development of lumbar pedicle screws and instrumentation since 1980s, lumbar posterolateral fusion (PLF) and posterior lumbar interbody fusion (PLIF) are now commonly performed for lumbar intervertebral fusion with lumbar nerve root decompression. However, since 1994 transforaminal lumbar intervertebral fusion (TLIF) has been recognized as an alternative lumbar intervertebral fusion operation in Europe and the United States. Representative indications are lumbar disc disease, lumbar degenerative spondylolisthesis, and lumbar degenerative scoliosis, and it enables posterior lumbar intervertebral fusion through an ipsilateral intervertebral disc portal. We reported the indications, surgical procedure, and surgical outcome of TLIF in this paper, and we discuss about lumbar fusion surgeries based on a review of the literature.
