　抄録：今回，われわれはAce社製captured hip screw内固定後に偽関節となり，再転倒しプレート下端で骨折を来した症例に対してWagner revision stemを用いた再手術を経験した．症例は86歳女性，81歳時，右大腿骨転子部骨折に対してcaptured hip screwによる内固定を施行した．1本杖歩行が可能であったが，自宅台所で転倒し，右股関節部に疼痛が出現し，歩行困難なため来院した．単純X線写真でプレートの脱転，プレート下端に骨折を認めた．再整復は困難であり，通常のステムでは対処できないと判断し，Wagner revision stemによる人工骨頭挿入術を施行した．術後5カ月を経過した現在，1本杖歩行可能であり，単純X線写真ではステムの沈下，弛みを認めず，術後経過は良好である．

　We report revision surgery with the Wagner revision stem to a 86-year-woman of whose trochanteric region of the femur failed to unite after internal fixation using Ace captured hip screw, and who sustained a new fracture at the distal end of the plate. Since open reduction and internal fixation would have been difficult, we decided to perform hemiarthroplasty with the Wagner revision stem. Five months after surgery, the results have been favorable, and the patient is able to walk with a cane. Radiographs have shown no sinking or loosening of the stem.
