下顎骨縦割経咽頭アプローチにより摘出した軸椎悪性腫瘍の2例

Mandible Splitting Approach for Total Resection of Malignant Tumors in the Axis. Report of Two Cases

　抄録：下顎骨縦割経咽頭アプローチにより肉眼的全摘を行い得た軸椎原発性悪性腫瘍の2例を経験した．症例1は63歳男性で診断は脊索腫，症例2は27歳男性で間葉系軟骨肉腫であった．後方アプローチによる後頭骨頚椎固定を行った後，症例1では2期的，症例2では1期的に下顎骨縦割アプローチにより腫瘍を全摘した．症例2では斜台からC3までの前方固定術も行った．症例1は術後6カ月で他の理由により死亡，症例2は術後8カ月の段階で再発はなく経過良好であった．本アプローチは良好な視野が得られることから，軸椎原発性悪性腫瘍の全摘に有用な方法である．

　A report of two cases with primary malignant tumor in the axis successfully removed using a mandible splitting approach. Case 1 was a 63-year-old male with chordoma and underwent staged surgery consisting of posterior occipito-cervical fixation and anterior tumor resection. Case 2 was a 27-year-old male with mesenchymal chondrosarcoma and underwent same-day anterior and posterior surgery. Case 1 died of an unrelated cause at six months after surgery and Case 2 were disease free at follow-of 8months after surgery. The mandible splitting approach can provide a wide surgical field, making gross total removal of primary malignant tumor in the axis possible.
