　ループス抗凝固因子が陽性で，脳梗塞を発症した悪性関節リウマチ（以下，MRA）の1例を報告した。

　15〜16年来，慢性関節リウマチ（以下，RA）として加療を受けていた68歳の女性が，右片麻痺を主訴に当院を受診した。頭部CTにて左半球に低吸収域を認め，脳梗塞の診断で入院となった。RAの関節症状に加えて，低補体血症，間質性肺炎，心膜炎の所見が認められたため，MRAと診断された。また凝固検査にて，ループス抗凝固因子が陽性であった。一般に，MRAの中枢神経障害は稀であり，その病因として脳血管炎が考えられている。本例では，脳血管障害の危険因子はみられず，脳血管炎を示唆する所見も認められなかった。本例の脳梗塞の病因としてループス抗凝固因子の関与が考えられ，MRAの脳血管障害の病因を考えるうえで重要な症例と思われ報告した。

We reported a case of malignant rheumatoid arthritis (MRA) with cerebral infarction associated with a possible cause of lupus anticoagulant.

The patient was a 68-year-old woman who had recieved treatment for rheumatoid arthritis (RA) from 15 to 16 years ago. She consulted to our hospital with a major complaint of right hemiplegia. Brain CT revealed a low density area in the left hemisphere. She was diagnosed as cerebral infarc-tion and hospitalized. Since she was noted to have hypocomplementemia, interstitial pneumonia and pericarditis, she was diagnosed as MRA.
