　Amantadineの投与によって意図性，間代性保続が改善〜消失した3症例を報告した。症例1は皮質下に病変の主座をもつ亜急性脳症後遺症，症例2,3は皮質下の多発性梗塞による脳血管性痴呆で，全例のPETまたはSPECTで前頭葉優位の血流代謝低下が認められた。投与薬剤はamantadine hydrochloride 150mg単独，または200mgとtrihexyphenidyl 10mgとの併用であった。症例1では保続とともにutilizationbehavior，汎性注意障害も改善した。症例2,3では汎性注意障害などの変化なしに保続の改善のみがみられた。Dopamine系賦活作用をもつamantadineに対するこのような反応は，保続の機序の一部に中脳—辺縁・前頭皮質dopamine系の機能低下があることを示唆している。

We report here 3 patients who showed marked improvement of clonic and intentional persevera-tions following the administration of amantadine. Patient 1 had a subacute encephalitis of unknown etiology. Magnetic resonance imaging documented lesions predominantly in the bilateral putaminal and frontal subcortical areas, and positron emission tomography revealed frontal glucose hypometa-bolism. Daily amantadine hydrochloride of 200mg with or without 10mg of trihexyphenidyl, reduced his clonic and intentional perseverations.
