定位脳手術のための加圧空気脳室撮影

POSITIVE PRESSURE PNEUMOVENTRICULOGRAPHY FOR STEREOTAXIC SURGERY

緒言

　定位脳手術にさいしてすぐれた手術効果を期待し，しかも合併症を避けるためには最も有効なtarget pointを選択し，十分かつ最少限の破壊を行なうことである。

　そのためにはまず解剖学的に正確にtarget pointを決定することが第1である。一般に大脳基底核付近にtarget pointを求める場合にはレ線上に造影された第3脳室壁に見られる特定の部位，たとえばanterior com—missureやposterior commissureなどを基準点として，これよりtarget pointが計測されている。したがつて定位脳手術時に第3脳室がいかに正確に造影されているかは手術効果を左右する重要な因子となる。

For the purpose of visualizing the third ventricle for stereotaxic surgery positive pressure pneumo-ventriculography was performed in 23 cases and the following satisfactory results were obtained :

1) On all occasions, the third ventricle was visualized completly by positive pressure pneumo-ventriculography without changing the position of supine patients at sterotaxic surgery.

2) Fifteen cc of air introduced into the lateral ventricle was sufficient enough to visualize the third ventricle and the inner pressure of the lateral ventricle at that time was under 12 mmHg.

3) The side effects due to increasing intraventri-cular pressure were all mild and disappeared im-mediately after release from pressure.
