I．緒言
近年，脳神経外科領域における各種脳疾患の病巣診断については，種々の補助診断法の確立ならびに改善により，十分，満足すべき成果をあげていることは周知のところであり，就中，脳動脈撮影法の普及は，脳血管性疾患ならびに脳内のspace taking lesionの診断にあたつては最も強力な武器となつている。
しかるに，この脳動脈撮影によつても，その原因を明らかになしえない脳血管性疾患に遭遇する場合もときにあり，特に，脳内微小血管の奇形性病変により惹起される脳内血腫の場合には，なかなか，その微小変化を立証することが困難であり，したがつて，従来，臨床的に，出血の誘因ならびに原因をり明らかになしえない脳内出血を"特発性脳内血腫"と呼称し，一つの特別なclinicalentityに含め，主として，病因論的な立場から検討されてきている。
A 8-year-old boy was admitted with rapid in-creased intracranial pressure ; headache, vomiting, chocked disk and consciousness impairment, whose clinical symptomes as well as angiographic and electroencephalographic findings suggested us the existance of space taking lesion at right cerebral hemispare.
Exploratory craniotomy disclosed the hematoma at the subcortex of right temporo-parietal region and reached the diagnosis of spontaneous intracere-bral hematoma in this case.
Etiological and pathognomotical considerations about spontaneous intracerebral hematoma is pre-cisely discussed.
