“もの忘れ”外来を受診した高齢者に対し，Frontal assessment battery（FAB）を施行し，その検査の意義について検討を行った。対象は“もの忘れ”外来を受診した高齢者82名（男性31名，女性51名）で，年齢は61～91歳（平均年齢74.9±5.8歳），平均教育年数は10.8±2.6年（6～16年），「もの忘れ」に気付いてから受診までの期間は平均22.2±21.9カ月であった。FAB日本語版に加え，Mini-mental state（MMSE）やRaven's Colored Progressive Matrices （RCPM）などの神経心理学的検査を施行した。

　認知症と診断したものは59名で，アルツハイマー病28名，前頭側頭型認知症14名，血管性認知症17名であった。残りの23名中，Mild cognitive impairment（MCI）は13名で，他の10名は異常がない（健常群）と思われた。FAB得点は健常群，MCI群に比べ，認知症の各群で顕著に低下を認めたが，健常群とMCI群の間に差を認めなかった。FABとMMSE，RCPMとの間に高い相関を認めたが，MMSEやRCPMで異常を認めず，FABで異常を呈するものも多くみられた。本検査の異常が前頭葉病変そのものを意味するものではないが，短い診療時間内に簡便に前頭葉機能をスクリーニングできる検査として有用と思われた。

To investigate the usefulness of the frontal assessment battery （FAB） as a brief assessment for diagnosis in patients with dementia, we examined cognitive function using the FAB, Mini-mental state examination （MMSE）, Kana-hiroi test, and Raven's colored progressive matrices （RCPM）. Subjects were 82 senile patients having amnesia as a complaint including 28 Alz-heimer's disease（AD）, 14 frontotemporal dementia（FTD）, 17 vascular dementia（VD）, 13 mild cognitive impairment（MCI）, and 10 healthy persons. We discussed the difference of scores in different types of dementia. In the FAB, the scores of healthy persons and MCI were much higher compared with the scores of all types of dementia. There were high correlation between FAB and MMSE, Kana-hiroi test,a nd RCPM. Although some subjects showed normal score in MMSE and/or RCPM, they showed low score in FAB. Based on this, FAB is efficient for the differential diagnosis of dementia as brief screening tool.
