抄録

　摂食障害は，遷延例，難治例が多く，十分な回復のないまま結婚，出産に至り，育児問題を抱えるリスクが高いことが予想される。我々は，育児問題をかかえた摂食障害10症例について，出産前後の摂食障害症状の経過，育児問題の内容と経過，具体的な援助方法について検討した。摂食障害患者の出産・育児におけるリスクとしては，1)妊娠中の身体変化への適応困難，2)依存をめぐる葛藤の増悪，3)強迫性，完全主義傾向の強化と抑うつ，4)子どもとの関係性にまつわるコントロールの問題，5)母子間のコミュニケーション問題，6)喪失体験が考えられた。精神科，産科，小児科の医療連携から，地域保健サービスにつなげる支援が重要であると思われた。

　Eating disorders among children are associated with a high risk of parenting difficulties. We evaluated 10 cases of eating disorders that involved parenting difficulties. The problems included 1) resistance to the change of body shape, 2) conflicts related to dependence, 3) obsession, perfectionism, and depression, 4) problems faced by parents in controlling their child, 5) communication between mother and child, 6) experience of loss. We conclude that it is important for psychiatrists to work in closer cooperation with obstetricians, pediatricians, and the local health staff.
