　本研究の目的は，精神科急性期治療にかかわる医療機関の治療コストと診療報酬収入を比較することである。地域的に異なる，急性期機能を担う3つの精神科病院の医師，看護師，精神保健福祉士からなる医療チームに事例を提示し，直接ケアについてヒアリング調査を行った。その結果，①隔離室を使用するような精神科急性期患者の入院1日目の収支は，どのような施設であっても赤字となる可能性が高いこと，②診療報酬基準として類似の医療機関を対象に選んだとしても，医療機関ごとに直接ケアの内容が大きく異なることが明らかになった。入院初期に十分な直接ケアの投入を可能にする診療報酬のあり方に関する議論に寄与できる結果を得た。

　The main objectives of this study are to examine on the relationship between costs and reimbursements for acute psychiatric inpatient care, in order to provide an empirical evidence for the current debate on reimbursement rates for psychiatric care. We interviewed psychiatrists, nurses, and psychiatric social workers (PSW) employed at psychiatric hospitals in three different locations. Our findings are as follows：(1) for patients requiring intensive acute psychiatric care, such as seclusion, financial loss was observed on the first day of hospitalization all three hospitals；and (2) the inpatient care significantly varied even among similar acute psychiatric hospitals. These results contribute to the discussion on the schedule medical fee revision that proposes more intensive care input at the early stage of psychiatric inpatient care.
