　右側副伝導路の部位診断の目的で右冠動脈内マッピングを行い，その有用性について検討した．右用6F Judkins catheterをguiding catheterとして0.014inch hi-torque floppyguide wireを先端以外は2.7F tracker infusion catheterにて電気的絶縁状態にして右冠動脈内に挿入した．guide wireの先端より単極誘導で明瞭な右冠動脈内の心房および心室電位が得られた．右室ペーシングや頻拍発作時における逆行性心房波の右冠動脈内マッピングから右側副伝導路の存在部位を診断しえ，これは術中マッピングとほぼ一致した．左後中隔と右後中隔との副伝導路の鑑別，複数副伝導路の診断にも有用であった．しかし，比較的近接している複数の副伝導路の診断および右冠動脈の解剖学的劣位例では限界があった．右冠動脈内マッピング法は簡便かつ安全に短時間で施行でき，右側副伝導路の部位診断に有用であった．

Intracoronary mapping of the right coronary arterywas performed to diagnose the location of the rightaccessory pathways in the Wolff-Parkinson-White syn-drome.A 6F Judkins right catheter was used for guidingthe catheter,and a 0.014 inch hi-torque floppy guidewire was covered,except for the tip,by a 2.7F trackerinfusion catheter and inserted into the right coronaryartery.The intracoronary electrogram was recorded bythe tip of the guide wire.The location of the rightaccessory pathways was diagnosed from the earliestretrograde atrial activation during the right ventricularpacing or AV reentrant tachycardia.
