　症例は47歳，女性．1976年12月に慢性関節リウマチで発症，1989年7月から悪性RAで加療を受けていた．1991年8月中旬から，労作時呼吸困難が出現し，右心不全を認めたため入院．心電図は著明な右室肥大を呈し，Swan-Ganzカテーテル法で肺動脈収縮期圧100mmHgと著明な肺高血圧を認めた．4年前の心電図は正常範囲であった．原疾患および肺高血圧，右心不全に対して積極的に加療したが約8カ月後に死亡した．慢性および悪性関節リウマチでは少数例に肺高血圧を合併する例がある．それらの症例の予後は極めて悪く，早期診断，治療が必要であり，定期的に心肺機能を検査することが重要と考え報告した．

A case of malignant rheumatoid arthritis with pulmo-nary hypertension was described. The patient was a 47-year-old woman who had been treated as having rheumatoid arthritis since December 1976, and then malignant rheumatoid arthritis since July 1989. She has been troubled by dyspnea on exertion since the middle of August 1991, and was admitted to our hospital on September 24th, 1991. Physical examination showed signs and symptoms of RVH and right heart failure. Laboratory deta (chest x-ray, ECG, UCG) also support-ed the diagnosis of RVH and marked pulmonary hyper-tension.
