心室性期外収縮の左室局所壁運動に及ぼす影響

The influence of ventricular ectopic beats on left ventricular wall motion

　心筋の電気生理学的異常によって，心室の機械的特性は大きな影響を受ける^1〜4）。心室性期外収縮に際して駆出能が低下する原因は主に，心房のbooster actionの喪失によると考えられてきた^5〜7）。しかし，心室性期外収縮は心室全体が脱分極するに要する時間を有意に延長させ，心室収縮にasynchronous patternを生じさせる^8）。また，心筋の局所機能は心室性期外収縮の発生部位と密接な関係を有する^9）。臨床的に，このような心室性期外収縮に伴う左室局所の運動異常が駆出の低下とどのような関係を持つかは，重要なポイントであり，心エコー法が開発されて以来，電気生理学的変化と左室壁運動に対する影響がくり返し検討されてきた^10〜12）。しかしながら，これらの研究は，現在のところ定量的というより定性的な検討で終わっている。

　心室ペーシングによる局所壁運動の変化は，開胸犬に超音波ディメンジョンゲージを装着し，局所左室セグメント長を測定した実験で検討され^13），局所セグメント長は，反対側のreciprocal interactionにより強く影響されることが示唆された。ヒトにおける心エコーを用いた局所壁運動異常の検討の多くは，壁厚変化により行われているため，これらの臨床データを理解するためには，局所壁厚を測定する実験の方がよりふさわしい。したがって，我々は，慢性犬で左室壁厚を測定し，心室性期外収縮の際のその収縮動態の変化を刺激部位と連結時間との関係から検討した。左室全体の機能として，1回駆出量を測定し，局所の異常との相関を検討した。

Effects of premature ventricular contration on the regional wall thickening pattern were studied in conscious dogs. A pair of ultrasonic crystals was implanted in the anterior and posterior walls to measure wall thickness. Aortic flow was measured with electromagnetic flow probe. After control re-cording at the resting state, premature contractions were produced randomly by stimulating the left atrium, and the anterior and posterior walls of the left ventricle. In premature ventricular contractions with identical coupling intervals, thickening characteristics were more markedly altered as evidenced by the deformation in the regional pressure-wall thickness loops in regions closer to the stimulation electrode. There was more prono-unced isovolumic thickening in beats with shorter coupling intervals. Thus, the pump function is depressed in premature ventricular contraction in part due to the increased ratio of wall thickening before the opening of the aortic valve.
