要旨●自己免疫性胃炎（AIG）の報告の増加に伴い，H. pylori感染が胃炎所見を呈するためAIG診断を困難にすることが問題となっている．H. pylori感染が，AIGの臨床所見，内視鏡所見に及ぼす影響と問題点，AIGとH. pylori感染との関連について検討した．AIGをH. pylori感染状態（H. pylori関連群と非関連群）に分けて検討すると，H. pylori感染を合併することでAIGの血清学的所見（血清ガストリン値や血清ペプシノゲン）や内視鏡所見に影響を及ぼすことが明らかとなった．H. pylori感染を合併したAIG症例の長期経過を追跡していくことで，H. pyloriとAIGの関連について新たに解明できる可能性がある．

　Diagnosing H. pylori（Helicobacter pylori）infection is difficult in patients with AIG（autoimmune gastritis）. In this study, we report the effects and problems related to H. pylori infection on the clinical and endoscopic AIG findings, and the association between AIG and H. pylori infection. We categorized AIG based on H. pylori status as H. pylori-related and non-related groups. H. pylori infection affected the serological findings, such as serum gastrin and pepsinogen levels, and AIG endoscopic findings.

　Follow-up studies on AIG cases associated with H. pylori infection may provide new insights into the association between H. pylori and AIG.
