要旨●代表的なリンパ球浸潤胃癌としてEBV（Epstein-Barr virus）関連胃癌に着目し，著明なリンパ球浸潤を伴った自験胃癌症例をEBV感染の有無でEBV関連胃癌12例23病変とEBV非関連胃癌81例83病変に大別し，その臨床病理学的特徴と内視鏡所見を比較検討した．EBV関連胃癌は非関連胃癌と比較して同時性多発病変が多く，胃噴門部・胃体部に好発し，通常観察では境界不明瞭な発赤調の病変が多かった．NBI併用拡大観察所見ではabsent MS pattern，irregular mesh pattern，corkscrew patternが特徴的であり，病理学的所見を反映するものと考えられた．以上よりEBV関連胃癌の診断にはNBI併用拡大内視鏡所見が重要と考えられた．

　This study focused on EBVGC（Epstein-Barr virus-associated gastric cancer）as a typical gastric carcinoma with lymphoid stroma. We compared the clinical pathological features and endoscopic findings of 23 lesions in 12 patients with EBVGC observed in our hospital and 83 lesions in 81 patients with EBV negative gastric cancers. Most EBVGC were located in the proximal stomach, manifesting as reddened lesions with indistinct margins. Further, there were many cases of synchronous multiple carcinogenesis. Microscopic analysis showed moderate-to-poorly differentiated adenocarcinoma. Narrow-band imaging with magnifying endoscopy of the EBVGC showed absent micro surface pattern, irregular mesh pattern, and corkscrew pattern. Therefore, it is essential to perform narrow-band imaging with magnifying endoscopy for EBVGC diagnosis.
