How to Take a Biopsy from a Tiny Gastric Lesion

要旨　生検は癌の確定診断に必要な手技であるが，5mm以下の微小病変から複数の生検を採取すると，病変が分断されてしまい，治療時の正確な範囲診断が困難となる．また，1か所のみの生検でも病変の中央から採取すると病変が消失してしまうことがある．微小病変に対する生検は，病変の欠損が最小限となるように，病変の辺縁から小生検鉗子で1個のみ採取するのが原則である．したがって，微小病変では生検の至適部位がわずかであり，適切な生検を行うには，まず病変を正確に診断し，適切な生検採取部位を同定する診断力が必要である．次に，標的部位から正確に生検を採取する精密な技術が必須である．生検が不適切な部位から採取されると正しい診断が困難となるため，視野確保が不十分な状態での生検採取は慎むべきであり，先端フードを用いた再検にて正確な生検を試みるべきである．

　Biopsy is necessary for the diagnosis of cancer. However, if multiple biopsies were taken from a tiny lesion（≦5mm）, the lesion might disintegrate and the diagnose of lateral extension would become difficult. Biopsy has a potential to cause the cancer to appear to disappear, even if only one biopsy is taken. Therefore, the selection of biopsy site and size is important to prevent apparent disappearance after biopsy. Therefore, only one biopsy should be taken from the edge of the lesion by small biopsy forceps. The optimum area for taking a biopsy from a tiny lesion is very small. Therefore, the endoscopists have to have the skill to take a biopsy from an adequate site. If the biopsy is taken from an inappropriate site, it would be difficult to make a correct diagnosis and decision. A transparent hood makes it easy to take a precise biopsy. Therefore, re-examination with a transparent hood should be considered if the approach to the tiny lesion is difficult.
