Diagnosis of Anorectal Diseases

　直腸肛門部の疾患は，心理的に差恥心の強い患者にとっては，医師の診断をできるだけうけないですまそうとする疾患の1つである．したがって直腸肛門専門の医院は，あまり人通りの多いところには入口を作らないという心遣いをしているところもある．事実，患者自身から聞いたことであるが，診療所の前を2度や3度は通りすごしてみて，最後に決心をかためて門を入ったというのである．

　もう1つは，肛門部は自分では見えない部分であり，糞便の出るところでもあるので不潔なところであるという観念が固定して，少々不潔にしていても，あまり気にしないという傾向がある．

　Because lesions in the anorectal region can be seen and palpated directly their diagnosis is not difficult provided that visual inspection and palpation be done with accuracy. However, one should be careful not to let the patient feel too self-conscious and the examination must be done with sympathy and consideration.

　Digital examination of the anal region must be done as gently as possible and yet fingers should be inserted deep enough to reach the upper part of the rectum. Otherwise, any possible changes in this segment are in danger of being overlooked.

　Great care must also be taken not to pass over the fact that lesions in the anal region could be local signs of other more general disorders such as syphilis, Crohn's disease or ulcerative colitis. In romanoscopic diagnosis it is essential to avoid any possible iatrogenic lesions liable to be caused by the examination.
