胃内回虫症

GASTRIC ASCARIASIS

Ⅰ．はじめに

　この特集の中に「胃内回虫症」が加えられて，つくづくと疾病の変遷を感じさせられた．

　戦前のわが国は「寄生虫王国」というあまり有難くない代名詞まで与えられた程で，とくに回虫の寄生率は全国的に高率であった．

　終戦直後から昭和23年頃は岩田繁雄教授のいう「敗戦症候群」の名称で代表されるように都市でも69～92％の高率に回虫感染がみられ¹⁾，寄生虫学関係の論文も激増している．

　しかし，第1表²⁾にしめすように昭和25年頃から食生活の改善，強力な駆虫剤の開発，集団駆虫の徹底などの効果が芽生えて回虫寄生もだんだんと減少し^3)4)5)6)，昭和30年頃には全国平均30％，さらに昭和39年には6.8％と激減している⁷⁾．

　現在，都市では文字通り「珍らしい疾患」になってきたし，医学生や若い医師の中には回虫卵をみたことのない方もあると思われるが，山村地帯では未だに13～50％の高率に感染している地域があり，その撲滅対策が種々検討されている^8)9)10)．

　われわれの観察し得た胃内回虫症も岡山県北部の山岳地帯で，胃精密X線，内視鏡の進歩した現在では機会が与えられるならば，その病態もより詳細に観察することができると思う．

　A 39 years olcl female complaining of epigastric pain, was referred to the authors' clinic. An excavation of irregular shape found at X-ray examination on the anterior wall of the greater curvature opposite to the gastric angle was studied by gastrocamera as well. Unexpectedly, an Ascaris was visualized in the antrum in addition to the excavation as was already noticed by X-ray film as well as abrupt cessation of mucosal folds around the lesion. The patient underwent gastrectomy under the tentative diagnosis of Ⅱc type gastric cancer. No Ascaris was found at operation as anthelmintics had previously been administered.

　There have been few reports so far about the Ascaris in the stomach as confirmed by endoscopy. Its discovery was always accidental.

　The infection rate of Ascaris is on an average about 5% in this country. In urban areas few people now have ascariasis, but in some rural districts there still remain many people who are victims to it. Careful endoscopic examination of the stomach will, it is hoped, reval some hitherto unknown aspects of gastric ascariasis in future.
