要旨　患者は56歳，女性．排便時の便表面への血液の付着を主訴として入院．大腸内視鏡検査にて直腸前壁に表面平滑で，血管透見像を有する山田Ⅱ型の中央に陥凹を伴う隆起病変を認めた．病変からの生検標本の組織学的検索の結果，良性リンパ濾胞性ポリープと診断され，経肛門的腫瘍摘出術を施行した．切除標本の病理組織学的検索では，病変は胚中心を有する正常なリンパ濾胞の増殖により形成されており，良性リンパ濾胞性ポリープと診断された．術前の超音波内視鏡検査では，病変は第2，第3層に均一な低エコー領域として描出され，エコー上からは悪性リンパ腫との鑑別は困難であった．直腸の良性リンパ濾胞性ポリープは本邦ではまれであり，自験例は15例目であった．

　A 57-year-old female was admitted to our hospital, complaining of rectal bleeding. Double contrast study of rectum showed the hemispheric polypoid lesion with smooth surface. Endoscopy showed a submucosal tumor with central depression in the rectum. The biopsy specimen taken from this lesion showed proliferation of normal lymphocytes with forming lymph follicles. Thus, this lesion was diagnosed as benign lymphoid polyp of the rectum. Endoscopic ultrasonography of this lesion showed well-demarcated hypoechoic area in the second and third layers.

　Transanal local resection was performed. Histologically, the polypoid lesion was composed of proliferation of lymph follicles with germinal centers in the submucosal layer in the resected specimen. Fifteen cases of benign lymphoid polyp of the rectum have been reported in Japan.
