　胆囊病変には，炎症，結石，良性腫瘍，悪性腫瘍など種々のものがある．この中でも，癌は特に最近注目されてきている．その理由として，臨床側での胆囊病変に対する診断学が最近飛躍的に進歩してきたこと，これにより予後の良好な早期胆囊癌や小さい胆囊癌が発見されていること，迅速診断や直視下生検診断の必要性が急増していること，早期癌の発見により癌の発生母地が研究課題となりつつあること，などが挙げられよう．

　したがって，本稿では胆囊癌に関する，最近の病理学的知見；（1）胆囊癌の肉眼的特徴，特に早期胆囊癌の特徴，（2）胆囊癌の組織学的診断と鑑別診断，（3）胆囊癌細胞と胆囊粘膜上皮細胞の産生粘液とCEA，および（4）胆囊癌の発生母地，について述べることにする．

　We studied the subjects on carcinoma of the gallbladder, that is, macroscopic characteristics, histological criteria and differential diagnoses, mucin and CEA, and histogenesis, using 42 cases of gallbladder carcinomas.

　The great macroscopic characteristic of the carcinoma was villous, papillary and roughly nodular surface of the elevated or flat lesions. Carboxymucin was predominant in the carcinoma. CEA was positive in 82% of the cancer cases. It was positive in 48% of cases with chronic cholecystitis, where it was limited to very small areas of some metaplastic mucosa and immature regenerating cells of proper gallbladder epithelium.

　A great majority of the carcinomas had severe metaplastic change in the surrounding mucosa. However, the precise histogenesis of the gallbladder carcinoma should be studied with early carcinomas or microcarcinomas.
