十二指腸球部の早期悪性リンパ腫の1例

Early Non-Hodgkin's Lymphoma. of the Duodenal Bulb, Report of a Case

要旨　患者は，65歳，男性．健診の目的で上部消化管内視鏡検査を行ったところ，十二指腸球部にびらん性病変が認められ，鉗子生検で悪性リンパ腫の疑いと診断された．半年後の第2回目鉗子生検で悪性リンパ腫と診断された．切除標本では，襞集中を伴う浅い陥凹性病変とその周辺に黄白色の腫大粘膜が認められた．組織像では，生検によるUl-Ⅱの小潰瘍瘢痕を認めたが，肉眼的陥凹部や粘膜腫大部には非腫瘍性リンパ濾胞の増生があり，その周辺（marginal zone）から腫瘍が発生していた．腫瘍は中細胞から成るB細胞型悪性リンパ腫で，深達度smまでの早期悪性リンパ腫であった．本例は，ごく早期の十二指腸marginal zone lymphomaとして世界でも最初の報告と思われる．

　A 65-year-old man visited our center for a medical examination. Endoscopic examination showed a small erosive lesion with swollen mucosa at the duodenal bulb. Biopsy speciman yielded the findings suggestive of malignant lymphoma. The second endoscopic examination and biopsy, performed 6 months later, definitively revealed malignant lymphoma.

　The resected specimen showed a shallow depression surrounded by swollen mucosa. Histologically the marginal zone is positive for the tumor tissue composed of many non-neoplastic lymph follicles. The tumor of medium-sized B-cell lymphoma spread diffusely involving the submucosa.

　This case, early non-Hodgkin's lymphoma of the duodenum, seemed to be the first as marginal zone lymphoma of the duodenum.
