回虫症の1例―十二指腸乳頭開口部に迷入した回虫の内視鏡的観察

Ascarides Migration into the Orifice of the Vater's Papilla, Report of a Case

　戦後都市農村の別なく，多くの感染率を示した回虫症は，「稀な胃病変」の症例特集¹⁾²⁾にとりあげられるほど急速に低下した．しかし，地域的にはいまだ相当の感染率を示しており²⁾¹⁰⁾，当院においても，腹部の痂痛ならびに不定愁訴で受診する患者の中に，回虫が原因であることがかなり多い．当院において，過去2年間の消化管X線検査および内視鏡検査で観察した回虫症は11例になる．このなかの興味ある1症例を中心に，当村の一地区を調査した寄生虫卵保有の現状などについて報告する．

　A 40-year-old woman was admitted to our hospital complaining of colic pain in the epigastrium.

　Eleven (lay:-2 after admission to the hospital (on Jan. 3, 1977), racliographic examination showed two or three ruunclwurm-like shadows in the duodenum.

　Two (lays later, we found part of a roundworm in the papilla by duodenoendoscopy (IF type B2). During the endoscopic observation, it rapidly migrated into the orifice of the Vater's papilla and disappeared from sight.

　Anthelmintics were aclministered, whereupon three rounclworms were passed into the stool. The abdominal pain ceased soon.
