要旨　近年の内視鏡診断技術の向上により早期の食道癌の発見数は飛躍的に増加した．そしてごく早期の癌であれば外科手術によらなくても内視鏡的治療が可能になった．その際，最も大切なことは癌の深達度診断である．最近では深達度mm，smの癌がそれぞれ3段階で術前診断および病理組織診断がなされている．従来のEUSでは粘膜下層の層構造の侵蝕程度からM-SM₁，SM₂，SM₃と区分してきた．それぞれの深達度診断の一致率は92.8％，71.4％，80％であった．現状ではEUSの診断能は標本割面のルーペ像を肉眼視する程度である．しかし最近開発された20MHzの周波数の高い装置では粘膜筋板の描出が可能となり，癌巣を直接描出し診断できるようになった．今後の展望として食道表在癌も超音波の画像から立体的に癌の量として表現されてくるであろう．

　Detectability of esophageal cancer has been improved by advanced endoscopic techniques. Endoscopic treatment has replaced partly surgical intervention in very early cases. An accurate diagnosis on the depth of cancer is prerequisite for instituting endoscopic treatment. There are three stages in each of mm and sm cancer in the current histological diagnosis. Conventional EUS is comparable to macroscopic view of a slice of specimen, making three-stage classification possible in mm and sm cancer into M-SM₁, SM₂, SM₃. Accuracy was 92.8%, 71.4%, 80.0%, respectively for each stage. Newly developed EUS with 20 MHz transducer is capable of detecting the muscularis mucosa of the esophagus. It is therefore expected that EUS provides histological findings even in cases of superficial cancer of the esophagus in the near future.
