要旨　患者は50歳，男性．胃潰瘍の経過観察日的で上部消化管内視鏡検査を受け，十二指腸第3部に径5mm弱の発赤調の陥凹性病変が発見された．組織生検にて高分化腺癌が強く疑われ，内視鏡的切除が実施された．病理学的には粘膜内に限局した径4mmの高分化腺癌0'-IIcと診断された．十二指腸第3部という十二指腸の深部で陥凹型の早期癌が発見されることは非常にまれで，内視鏡的に合併症なく切除された例としては4例目であり，貴重な症例と思われる．

　A 50-year-old man visited our hospital for an annual follow up of a gastric ulcer. Upper gastrointestinal endoscopic examination revealed a depressed lesion in the third portion of the duodenum. The endoscopically resected specimen showed type 0-IIc early duodenal cancer, measuring 4 X 4 mm in size. Histological examination showed a well differentiated adenocarcinoma in the surface mucosa. Depressed type early duodenal carcinoma in the third portion of the duodenum is raie and, including this case, there are only 4 cases reported in Japan of endoscopically resected depressed type early duodenal carcinoma.
