要旨　患者は51歳，男性．職域の胃検診で胃角部の不整を指摘され，精査目的で上部消化管内視鏡検査を受診した．十二指腸第2部下部に径10mm弱の粘膜下腫瘍様の隆起病変が認められた．初回の内視鏡検査時に生検を1か所実施したが，治療目的の内視鏡検査時には病変の形態がかなり変化していた．内視鏡的に切除した病変の病理学的検索では，粘膜下層に正常十二指腸粘膜が憩室様の形態を示しつつ迷入しており，それが粘膜下腫瘍様の形態を呈した原因と考えられた．また，腫瘍細胞は病変の粘膜表層に限局し，高分化腺癌0-Ⅱcと診断された．このような粘膜下腫瘍様の形態を示した早期十二指腸癌はわが国では報告はなく，貴重な症例と思われた．

　A 51-year-old man visited our hospital for upper gastrointestinal endoscopic examination because of abnormal findings in x-ray examination of the stomach. Endoscopic examination revealed an elevated lesion mimicking the submucosal tumor, in the second portion of the duodenum. The endoscopically resected specimen showed type 0-Ⅱc early duodenal cancer, measuring 8 mm in size. Histological examination showed well differentiated adenocarcinoma in the surface mucosa with a cyst lined by normal duodenal mucosa invaginated into the submucosal layer. Early duodenal cancer mimicking the submucosal tumor is the first case reported in Japan.
