要旨　1990年から2011年までに診断，治療を行った大腸SM癌516病変について検討した．内視鏡治療手技の進歩とともに2005年以降では，2004年以前と比較して内視鏡切除を施行する大腸SM癌が増加傾向にあるのと同時に，追加外科切除に回る症例も増加傾向にあった．2005年以降の経過観察例から，「大腸癌治療ガイドライン」による内視鏡治療後の方針に関するフローチャートは妥当と考えられたが，依然としてみられているover surgeryを今後減少させるためには，さらに詳細なリンパ節転移危険予測の検討をしフォローアップデータの蓄積を行って，オーダーメイドの治療方針決定に必要なデータを提示することが次のステップとして重要である．また，リンパ節転移因子のスコア化が有用である可能性が示唆される．

　During the period of 1990 to 2011, 516 SM carcinomas that were diagnosed and treated in our unit were investigated. With the advancement of endoscopic technology, endoscopically treated SM cancers as well as surgically treated ones increased after 2005 compared with the numbers before 2005. From analysis of the data after 2005, Japanese“Therapeutic Guideline for the colorectal carcinomas”were appropriate. In order to decrease the cases of oversurgery after endoscopic treatment that still happen even after 2005, it will be important as the next step to determine, pathological risk factors leading to lymph node metastasis. These should be analyzed in detail and data indicating “order-made treatment” should be presented after the analysis of accumulated follow-up results of observation of SM cancers after endoscopic and surgical treatment.
