要旨　腐食性胃炎は胃壁の伸展不良に加えて潰瘍，びらんなど粘膜面の変化が強く，スキルス胃癌との鑑別を要することがある．特に胃壁の伸展不良は受傷から時間が経つにつれて強くなり，鑑別に難渋することがある．粘膜所見を評価する際には内視鏡検査が有用であり，胃壁の伸展性を評価する際にはX線検査が有用である．また，酸・アルカリなどの曝露時間の違いから傷害の程度は異なるが，腐食性胃炎では咽喉頭や食道にも粘膜傷害が認められることも鑑別のポイントである．両者の鑑別に際しては，酸・アルカリなどの飲用や精神疾患の既往など正確な問診の聴取が重要となってくる．

　Corrosive gastritis presents ulcers and/or erosions with a rigid gastric wall due to strong mucosal damages, mimicking scirrhous gastric carcinoma. Endoscopic examination is suitable for evaluating mucosal damage and X-ray is suitable for recognizing rigidity of the gastric wall respectively. The degree of mucosal damage depends on the duration of exposure to damaging agents. The degree of rigidity increases with time. That makes it difficult to differentiate corrosive gastritis from scirrhous gastric carcinoma. One differentiating point is that corrosive gastritis has mucosal damage in not only the stomach but also in the pharyngolarynx/esophagus. A precise medical interview is necessary concerning the intake of acid or alkaline and any past history of psychiatric disease.
