要旨　病院受診者に対しては，内視鏡検査が臨床診断およびその範疇で行われる検診胃X線検査後の精密検査の第一選択として行われているのが現状である．しかし，内視鏡偽陰性例（見逃し例）の発生も少なからず存在しており，第一に偽陰性例を減らす努力と，第二に一定の割合で偽陰性が発生するとしたうえでの，病院受診者個人の胃癌発生のリスクや胃癌の多様性を考慮した適切な内視鏡検査間隔の設定が今後は必要であると思われる．

　Endoscopy is an initial diagnostic workup recommended for patients with any positive findings on upper gastrointestinal (UGI) study in hospitals. However, false negatives (i. e., missing disease) can occur on endoscopic examination and cannot be disregarded. Efforts to reduce the number of false negatives and the establishment of appropriate intervals of screening considering the incidence and variety of gastric cancer based on the probability of occurrence of false negatives will be indispensable.
