胃分化型SM1癌（垂直浸潤500μm未満）の診断―500μm以上と対比して―隆起型早期胃癌におけるX線・内視鏡診断

Radiological and/or Endoscopical Characteristics of Early Gastric Elevated Cancers

要旨　分化型SM癌の深達所見を9症例で検討した．SM浸潤500μm前後の鑑別は症例数が少なく検討不可能であった．SM浸潤1,000μm前後の鑑別は病変の厚みの評価が必要と思われた．そのためには従来から言われてきたSM中等度以深の深達度診断に加えて病変内の収縮・伸展で生じるわずかな凹凸変化にも注意を払う必要がある．今後は癌のSM浸潤を定量的に扱いながら検討を行う必要があるため，SM層の情報を積極的に収集するよう配慮された検査所見の蓄積および検討が必要である．

　We examined the findings about 9 early gastric cancers penetrating to the submucosal layer. It was difficult to study the difference of cancer penetration findings ; slight (within 500μm) and more because we had few cases suitable for this aim. It was necessary to evaluate the thickness of the cancer in order to distinguish submucosal penetration within 1,000μm from that exceding this depth. So, it is necessary to pay attention to the changing wave of the cancer surface according to expansion and contraction as well as conventional findings. We must accumulate findings about submucosal gastric cancers because quantitative data about submucosal invasion of early gastric cancers can provide knowledge needed for their treatment.
