　早期胃癌に対するEMRの一般的適応としては，“胃癌治療における施設間差をなくすこと”“治療の安全性と治療成績の向上を図ること”を考慮すると，胃癌学会が提唱するガイドラインの適応基準が妥当であると考えられた．一方，EMRの利点を鑑みると，必然的にその適応は拡大していくと思われる．そのための一括切除を目指した手技の開発・改良も進みつつある．今後，根治を念頭に置いた上で，根拠に基づいた適応の拡大を進めるため，臨床研究として行っていくべきであると思われる．

　The Japanese gastric cancer association has proposed the indication criteria for endoscopic mucosal resection (EMR) of early gastric cancer. It is as follows ; intramucosal cancer without ulcerative findings, histologically differentiated type, and less than 20 mm in diameter. This proposal is suitable as a minimal standard for general gastroenterologists. In the near future, These criteria will be expanded widely based on the results of analysis of lymph node metastasis and prognostic data after clinical trials. This will be supported by the development of new EMR techniques.
