要旨　胃に肉芽腫性病変を生じる疾患のうち，Crohn病以外の代表的疾患である，結核，サルコイドーシス，梅毒などについて，その形態学的特徴を含めた臨床像について概説した．いずれの疾患も胃粘膜に不整な潰瘍・びらんを主体とする病変を生じることが多いが，結核とサルコイドーシスでは多彩な形態を呈するためX線・内視鏡所見のみから診断することは困難である．一方，胃梅毒は幽門前庭部に好発し，X線では全周性漏斗状狭窄，内視鏡では易出血性の浅い不整形の多発潰瘍やびらんを呈し，副病変として胃体部に梅毒性皮疹類似の粘膜病変を伴うなど，X線・内視鏡の特徴的所見から診断可能なことが多い．肉芽腫性胃炎を生じる疾患はいずれもまれな疾患でありX線・内視鏡のみでは診断困難な場合も少なくないため，個々の疾患の臨床的特徴も含め鑑別診断を行っていくことが必要である．

　Granulomatous gastritis is characterized by the presence of granuloma within the gastric mucosa or submucosa. We described morphological and clinicopathological features of the representative diseases such as tuberculosis, sarcoidosis and syphilis.

　Gastric granulomatous lesions involved in tuberculosis and sarcoidosis are nearly always accompanied by mucosal erosion and multiple shallow ulceration. However, those morphological findings are so various that diagnosis is difficult by endoscopic or radiographic findings. On the other hand, gastric syphilis can fairly easily be diagnosed by endoscopic or radiographic findings because of the morphological features.

　Granulomatous gastritis is a rarely diagnosed disorder except when involved in Crohn's disease. In most other cases, the diagnosis depends on the clinical history, histological appearance, evaluation of other gastrointestinal and visceral lesions.
