要旨 患者は53歳，男性．胃透視ならびに内視鏡検査により胃体上部小彎前壁寄りに粘膜下腫瘍様隆起を指摘された．生検診断にて腺腫と診断されたが，高分化型腺癌を完全には否定できないため外科的切除が行われた．粘膜下腫瘍様隆起は主に粘膜層と連続する憩室様の嚢胞状組織で構成されており，嚢胞内には乳頭状あるいは腺管状に増殖する異型腺管が認められた．これら腫瘍の一部が粘膜陥凹から粘膜表面に露出していた．本例における異型腺管の細胞異型は軽度であり，幽門腺型腺腫と診断した．しかし軽度ではあるが構造異型を伴っていたことから，胃の多発性粘膜下異所性腺から発生した低異型度の幽門腺型腺癌の可能性も完全には否定できないと考えられた．
A 53-year-old man was admitted to our hospital for further examination of a submucosal tumor-like lesion in the stomach. Partial gastrectomy was performed because the endoscopic biopsy specimen was supposed to be adenocarcinoma. Gross findings of the resected specimen showed a submucosal tumor-like lesion, 20 mm in size. It was mainly located in the submucosal region of the fundic gland mucosa. It was pathologically diagnosed as a pyloric gland adenoma arising from the heterotopic gastric glands.
