要旨 患者は86歳，女性．他院で十二指腸球部の隆起性病変を指摘され，精査治療目的で当科入院となった．上部消化管X線および内視鏡検査で十二指腸球部上壁に結節状の山田Ⅳ型の隆起性病変が存在した．頂部からの生検では腺腫との診断であったが，focal cancerの存在も否定できず，完全生検目的のため内視鏡的ポリペクトミーを施行し，24×22×20mm大の標本を回収した．病理組織学的には腺腫の一部に高分化腺癌が存在するcancer in adenomaの症例であった．本邦では内視鏡的切除を行った早期十二指腸癌42例中，生検で癌陽性の症例は16例と少なく，完全生検を目的とした内視鏡的ポリペクトミーは第一義的な診断・治療手技として有効であると思われた．
A 86-year-old woman was admitted to our hospital because of the more detailed examination for a polypoid lesion in the duodenal bulb. Radiographic and endoscopic examinations of the upper gastrointestinal tract showed a pedunculated polypoid lesion with nodular surface and partial erythema. Endoscopic polypectomy was performed and the resected tumor was 24×22×20 mm in size. Histological examination revealed tubular adenocarcinoma restricted within the mucosal layer of tubular adenoma.
From the diagnostic and therapeutic points of view, we recommend endoscopic polypectomy for early duodenal cancer.
