 Since almost all gastric cancers develop from the background of Helicobacter pylori (H. pylori) infection of the gastric mucosa, H. pylori plays an important role in gastric carcinogenesis. Therefore, eradication of H. pylori may prevent the incidence of gastric cancers. In experimental studies using Mongolian Gerbils, the effect of H. pylori eradication was proven as a prophylaxis the gastric cancers. However, the results of recent randomized controlled studies have been very controversial. The effectiveness of H. pylori eradication in the prevention of gastric cancer has yet to be confirmed in clinical studies.

 On the other hand, prospective interventional clinical trials have confirmed that eradicating H. pylori is effective in suppressing the recurrence of gastric cancer in other areas after endoscopic treatment of early gastric cancer. Also retrospective multi-center surveys have indicated that the incidence rate of secondary gastric cancer in the group of endoscopically resected cases of early gastric cancer after H. pylori eradication is about one third of that found in the non-eradication group. These suggest that eradicating H. pylori and providing suitable treatment suppress the development of secondary early gastric cancer. Follow-up periods after successful eradication were less than10years, so it is too short a period to evaluate whether eradication prevents new occurrence. The results of these studies also indicate that H. pylori eradication may decrease the rate of gastric cancer growth.
