要約　目的：抗アクアポリン4（AQP4）抗体陽性視神経炎は重篤な視機能障害をきたしやすく，標準的な急性期治療を行っても十分な視機能回復が得られない症例が存在する。今回，視神経脊髄炎スペクトラム障害における再発予防療法として確立されているイネビリズマブを，急性期治療終了後早期に導入した1例を報告する。

症例：61歳，女性。左眼視野障害で受診し，抗AQP4抗体陽性であったことからAQP4抗体陽性視神経炎と診断した。ステロイドパルス療法2クールおよび血漿交換療法を5回施行した。Day 15時点で左矯正視力は0.04まで改善したが，中心部を含む視野障害が残存していた。急性期治療終了後早期に，再発予防の目的でイネビリズマブを導入した。その後，新たな再発はなく，経過観察中に視力および視野の改善を認めた。

結論：抗AQP4抗体陽性視神経炎の1例において，急性期治療後早期にイネビリズマブを導入し，その後の経過観察中に視機能の改善を認めた。自然経過や急性期治療の遅発的影響が関与している可能性があり，本症例における視機能改善の機序の特定は困難であるが，急性期治療後早期のイネビリズマブ導入は，一定の臨床的意義を有する可能性が示唆された。

Abstract　Purpose：Anti-aquaporin-4（AQP4） antibody-positive optic neuritis often leads to severe visual impairment, and some patients fail to achieve sufficient visual recovery despite standard acute-phase treatments. We report a case in which inebilizumab, an established relapse-prevention therapy for neuromyelitis optica spectrum disorder（NMOSD）, was initiated early after completion of acute-phase treatment.

Case：A 61-year-old woman presented with visual field disturbance in the left eye and was diagnosed with NMOSD-associated optic neuritis based on positivity for anti-aquaporin-4 antibodies. She received two courses of high-dose intravenous methylprednisolone followed by five sessions of plasma exchange. Although her best-corrected visual acuity improved to 0.04 by day 15, a central visual field defect remained. Inebilizumab was then initiated early after completion of acute-phase treatment for relapse prevention. No further relapse occurred, and both visual acuity and visual field gradually improved during follow-up.

Conclusion：In a case of AQP4 antibody-positive optic neuritis, inebilizumab was initiated early after acute-phase therapy, and visual function improved during follow-up. Although the mechanism underlying this improvement cannot be clearly determined, as it may reflect the natural course or delayed effects of acute-phase treatment, early initiation of inebilizumab may have clinical significance.
