Abstract Purpose:To evaluate the role of endothelial keratoplasty as a therapeutic procedure in patients for whom visual rehabilitation was not the primary goal.

Subjects and Methods:Four eyes with bullous keratopathy underwent Descemet stripping automated endothelial keratoplasty primarily for pain relief and infection control. The clinical backgrounds, visual acuity, corneal findings, and postoperative symptoms were retrospectively reviewed.

Results:Corneal edema improved and ocular pain resolved in all cases after surgery. Postoperative visual acuity remained limited, as preoperatively anticipated;however, suture-related corneal deformation and irregular astigmatism were avoided.

Conclusions:Endothelial keratoplasty may serve as a therapeutic corneal transplantation option for symptom control and ocular surface stabilization in selected cases in which visual improvement is not expected.
