・混合性結合組織病(MCTD)経過中に全身性エリテマトーデス(SLE)の免疫学的所見を認め,Sweet病を発症した1例を経験した.
・これまで報告ではMCTDとSLEに合併したSweet病を発症する症例は極めてまれであった.

A case of Sweet's disease association with immunological findings of Systemic lupus erythematosus during follow-up as Mixed Connective Tissue Disease

A 20-year-old female suffered from Raynaud's phenomenon, and a swelling of her fingers appeared 4 years ago. Blood tests revealed positive of anti-RNP antibody and any else negative of specific antibody at this time. She had been diagnosed as Mixed Connective Tissue Disease (MCTD) and observed follow-up for 3 years. At 8 month before admitting to our hospital, the positive anti-dsDNA antibody (79 IU/ml), low complement (C3 41.3 mg/dl, C4 6.2 mg/dl) and low white blood cell count (3,200/µl) in the blood test were detected without any clinical findings. She referred to emergency visit in any University Clinic with three days history of skin erythema and painful pustules in her the right neck, face with a fever over 40 degree. The histopathological findings of her skin lesions were consistent with Sweet’s disease, numerous neutrophil infiltrations in the entire dermis. Her skin lesions and systemic symptom improved promptly with systemic corticosteroid therapy.
