Abstract A 40-year-old male patient visited our hospital complaining the rash on the left buttock and the thigh, which he noticed three days after the operation of hemorrhoids. We initialy thought his eruption was induced by oral medicines or disinfectants, so we did patch test using these components and the Japanese standard allergens. As a result, the patch test showed a positive reaction only to the anesthetic allergen (caine mix) included in Japanese standard allergens. He had a history of using the commercial topical ointment frequently, and we supposed that he had been already sensitized to the local anesthetic by commercial antipruritic ointment. Finally we concluded his postoperative eruption was a systemic contact dermatitis, due to the local anesthetics which was used during the hemorrhoids operation.
