大腸腫瘍に対する超音波内視鏡診断―深部断端予測に基づく治療適応の最適化―

Endoscopic ultrasonography for colorectal tumors: Optimizing therapeutic indications based on prediction of vertical margin

要旨

近年，大腸T1癌に対するESDの増加に伴い，より精緻な術前診断が不可欠となっている。特に診断に苦慮する病変に対して，壁層構造を可視化するEUSはきわめて有用である。本稿では，EUSの実践的手技やピットフォールを概説するとともに，垂直断端（VM）の確保を術前に予測する「EUS-TFD分類」を紹介する。また, 粘膜下層の高度線維化や直腸局在などにおける画像と病理の乖離要因を解説し，過剰侵襲を回避するための診断的ESDを含めた，内視鏡治療適応の最適化に向けた最新の戦略を論じる。

　Precise preoperative diagnosis is essential for suspected cT1b colorectal cancer treated via ESD. Endoscopic ultrasonography (EUS) is an indispensable modality for visualizing transmural layers in borderline cases. This review outlines practical EUS techniques and pitfalls, and introduces the “EUS-TFD classification” for predicting a safe vertical margin (VM). By analyzing discrepancy factors such as severe fibrosis and rectal location, we discuss optimized therapeutic strategies, including the role of diagnostic ESD to avoid unnecessary surgery.
