ステロイド性骨粗鬆症に対するビスホスホネート製剤の効果

　ステロイド投与時の副作用として最も頻度が高いのが，骨粗鬆症およびそれに引き続く骨折である。高い骨密度でも骨脆弱性があり骨折を起こし，ステロイド投与者自身が積極的に治療しなくてはならない。現在，ビスホスホネートは海外のガイドラインにおいても本症の主軸となる薬剤として位置づけされており，本邦からの「ステロイド性骨粗鬆症の管理と治療ガイドライン2014年改訂版」においても推奨薬とされている。ビスホスホネートによる顎骨壊死や非定型骨折などが言われているが，ステロイド性骨粗鬆症の骨折リスク低減のベネフィットは大きく，治療を躊躇してはならない。

An increased risk of both vertebral and nonvertebral fractures has been reported with dosages of prednisolone or equivalent as low as 2.5-7.5 mg daily. The skeletal effects of glucocorticoids include both direct and indirect actions on bone that result in an early, transient increase in bone resorption accompanied by a decrease in bone formation, which is maintained for the duration of glucocorticoid therapy. Bisphosphonates are the front-line choice for prevention of fracture in glucocorticoid-treated patients and are recommended renewal GIOP guideline from Japan.
