要旨

　直接作用型活性化第Ⅹ因子阻害薬内服中の右室穿孔患者に対し，アンデキサネットアルファ（andexanet alfa：AA）を投与して外科的止血術を行った症例の麻酔を経験した。AAの投与に関して心臓血管外科医を始めとする関連部署と協議を行い，手術を無事に終了できた。このような症例では関連部署との協議が重要と考える。

　We describe the case of a patient with right ventricle damage for whom andexanet alfa was administered beginning before the surgery to avoid surgical hemostasis that could occur under the effect of a direct-acting factor Ⅹ inhibitor, Edoxaban. The patient was a 86-year-old woman whose vertricle damage was associated with a leadless pacemaker insertion. The surgery conducted to repair the ventricle damage was successfully completed after a thorough discussion among anesthesiologists and cardiac surgeons concerning the application of andexanet alfa.
