 Andexanet alfa is an expensive neutralizing agent that was recently approved as a direct-acting factor Xa inhibitor. It is not yet known whether the administration of andexanet alfa can cause heparin resistance during a cardiopulmonary bypass in cardiac surgery. We report a case in which andexanet alfa was administered prior to acute aortic dissection surgery for a 81-year-old man;64,000 units of heparin and 4,500 units of an antithrombin Ⅲ preparation were required before the activated clotting time exceeded 480sec.

 Although the surgery was completed without complications, we conclude that AA administration should be used with caution.
