We describe a case of difficulty in the extubation of a 82-year-old female patient due to the cuff adhesive structure of the tracheal tube used. At the first attempt to extubate the patient, the deflated cuff was caught in the patient’s subglottis and extubation was not possible. The cuff was inverted and glued in such a way that it folded. We observed that when the cuff was deflated, the area around the cuff deflected and expanded into a ring shape. This patient’s case emphasizes the need to take the potential necessity of further measures into consideration when a cuff adhesive structure and/or the tube size make extubation challenging.
