神経ブロックおよび在宅医療連携により高齢の両親による在宅看取りが可能になった終末期がんの1症例

A Patient with Terminal Cancer for whom Nerve Blocks and Home Medical Care Collaboration Enabled End-of-life Care at Home by His Elderly Parents

要旨

　神経ブロックを施行し，高度のがん疼痛が緩和され，適切な社会資源の導入で高齢の両親による在宅看取りが可能になった症例を経験した。適切な神経ブロックとオピオイド鎮痛薬の使用および社会資源の活用は，患者家族をも取り巻く全人的苦痛緩和に貢献できると感じた。

　We treated a 51-year-old male whose severe cancer pain was alleviated after the administration of nerve blocks. He had been transferred a to palliative care department and then underwent CT-guided nerve blocks. About 2 months after the nerve blocks, his pain had eased and he was discharged home. With the introduction of appropriate social resources, his elderly parents were able to provide end-of-lie care for him at home. This experience demonstrates that the uses of appropriate nerve blocks, opioid analgesics, and social resources not only contribute to alleviating patients’ pain；they can also help alleviate the pain of a patient’s family as a whole.
