はじめに

　高齢者の認知機能は，手術・麻酔に影響を受けやすい。The Nomenclature Consensus Working Groupは，術後12カ月までの認知能障害を周術期神経認知機能障害と定義している^1）。術後せん妄（postoperative delirium：POD）は，その中でもっとも早期，通常術後1週間までに発症する。PODは明確な診断基準があることから臨床研究も多く実施され，諸外国においては診療ガイドラインも作成されている。また，PODはその後の認知機能障害の発症と関連することが知られている。したがって，高齢者の術後長期的な認知機能を保護するため，POD予防対策の重要性は高まっている。PODの30％以上は非薬物的な介入により予防可能とされており^2），周術期チームは術前から術後を通した予防策を確立し，PODを減らすことで健康寿命の延伸を目指す必要がある^3）。

　PODの診断は，アメリカ精神医学会の “精神疾患の診断・統計マニュアル第5版” に基づいてなされるが，ベッドサイドではConfusion Assessment Method（CAM）などのツールが用いられる。股関節手術などの準緊急手術を受ける高齢患者はすでに認知症を合併していることも少なくない。認知症自体は，せん妄の強力な危険因子であり，特に認知症患者のせん妄（delirium superimposed on dementia：DSD）として近年注目されている。一方，認知症患者は環境の変化により，周辺症状（behavioral and psychological symptoms of dementia：BPSD）と呼ばれる知覚や思考内容，気分あるいは行動障害が生じやすい。日常診療において，POD，DSDおよびBPSDを鑑別することは困難である。

　本論文は，解明されつつあるPODの発症メカニズムや予測のためのバイオマーカーを概説し，高齢者の準緊急手術におけるPODの予防策に焦点を置いて論じる。

　The number of elderly people who may need surgical intervention will significantly rise as the population ages. Cognitive function is highly vulnerable to surgery and/or anesthesia；perioperative neurocognitive disorders, including acute postoperative delirium（POD）, are more frequent in the elderly. Evidence suggests a close relationship between the development of POD and the increasing risk of subsequent persistent cognitive decline, including dementia. Hence, preventing POD may improve elderly patients’ postoperative quality of life. On the other hand, POD pathogenesis is complex and related to the aging process, for which there is no specific treatment. Currently, non-pharmacological interventions are recommended with demonstrated dose-dependent effectiveness. Although urgent surgery is common among elderly patients, there are time constraints particularly before surgery. However, a multidisciplinary team should implement all possible measures during each phase of the perioperative period. Therefore, it is desirable to improve the quality of medical care for the elderly in daily clinical practice. Here, we present the current status of POD pathogenesis and putative biomarkers for early detection and will focus on the preventive strategy to reduce POD, especially in urgent surgery.
