外科系外来に麻酔科問診票を配置し手術予定患者の問題点を早期把握する術前評価システム

A Preoperative Evaluation System Using Pre-anesthesia Questionnaires in Surgical Outpatient Clinics in Order to Identify Medical Problems of Patients Scheduled for Surgery

要旨

　術前麻酔科外来のない当院で運用している，予定手術患者の問題点を早期に把握する術前評価システムを紹介する。外科系外来で手術決定した患者が記入した麻酔科問診票を麻酔科で受け取り，電子カルテ診察を開始する。麻酔科医は外科医に追加検査や併診，主治医受診日に麻酔科診察を依頼し，主治医，専門医と結果を協議する。早期介入により麻酔科と各科間の周術期リスクに対する情報共有，効率的な連携が可能になっている。

　We introduce a pre-anesthesia evaluation system in a hospital without an outpatient preoperative anesthesiology clinic, to enable earlier identification of the medical problems of patients scheduled for surgery. Patients scheduled for surgery fill out pre-anesthesia questionnaires in the surgical outpatient clinics. Anesthesiologists then receive the questionnaires and assess patients’ medical problems with electronic medical records. In the case of potentially high-risk patients, anesthesiologists request that surgeons add examinations or consultations with subspecialists or physical examinations by anesthesiologists on the same day the patients are seen by the surgeons for preoperative examinations. Anesthesiologists use the questionnaires as preoperative patient records, note the dates and results of the examinations on them, follow up on the preoperative status, and confer with both the surgeons and other specialists. This early preoperative evaluation system facilitates recognition of preoperative risks；promotes information sharing among anesthesiologists, surgeons, and other subspecialists；and ensures efficient collaboration.
